A FIRST ENQUIRY

Start small.
Ask what
you need to know.

You do not need to explain everything before deciding whether counselling might be a useful next step.

The enquiry form is hosted separately by Jotform. This page does not collect your answers or submit them on your behalf.

01 / MAKE AN ENQUIRY

A small first step.
A separate form.

Use the form hosted on Jotform to make an enquiry. You can review it before deciding what to share.

Choose a contact method you can safely access, and make your reply or voicemail preferences clear. Keep an initial enquiry brief; avoid unnecessary clinical histories, documents or another person’s private information.

Family therapy is for ages 11 and over. Suitability, consent, safety and the relevant arrangements for young people still need individual assessment.

Opening the form is not a booking, a guarantee of suitability or a promise of a reply within a particular time.

JOTFORM · EXTERNAL ENQUIRY FORM

Continue on Jotform.

The enquiry form is hosted separately on Jotform. Opening it does not send a message or book an appointment.

Information provided on the external form is subject to Jotform’s and the form operator’s arrangements, not this static website. Notification contents and delivery have not been independently tested. Read the form’s privacy information and contact arrangements before entering details.

The current privacy information is not a final practice privacy policy. If you are unsure how your information will be handled, clarify this before sending sensitive details.

Our optional wording guide below stays on this page. Its choices are not copied to the form.

02 / OPTIONAL WORDING GUIDE

What would you
like to ask about?

If finding the words feels difficult, use these general options to explore a helpful question and an example message. This guide is separate from the Jotform enquiry form.

Choices change this page only. The guide does not send, save or copy them to Jotform. Reset clears the choices; leaving or reloading the page resets the guide.

How the wording guide handles information
01 I’m considering…
02 First, I’d like to understand…

A useful first question could be: “Could you explain how you work, the practical arrangements, and how to decide whether counselling is a suitable starting point?”

A first message
is not your
whole history.

A brief enquiry can include a preferred name or pseudonym, the broad type of support you are considering and one safe way to reply. You can ask whether anonymous or pseudonymous contact is possible. There is no need to begin with a detailed clinical history, identity documents or another person’s private information.

Before sending anything sensitive, check the provider’s contact details, collection notice and privacy information. Choose a reply channel you can safely access, and say whether a message or voicemail is safe. Do not assume permission to leave a message has been given. You do not need a partner or family member’s permission to ask about support for yourself.

The enquiry happens on Jotform, not in this wording guide. Review the form’s own notices and communication arrangements before submitting. Sending an enquiry does not by itself establish a counselling relationship or confirm an appointment.

EXAMPLE WORDING

An example.
Not a sent enquiry.

This wording is made from the options on this page. It contains no personal details and is not sent to anyone.

Hello, I’m considering counselling and would like to understand how it works. Could you explain your approach, practical arrangements and how to decide whether the fit is right?

This example is not sent or copied to Jotform. To make an enquiry, use the separate hosted form. Opening or reading this example does not book an appointment.
